Welcome to the Public Domain Case Note Project!
Making case notes is not usually a therapist's favorite task. It is the purpose of this project to make that task easier. The next page contains a list of possible case notes. Check the ones you want, click on submit and those you selected are organized into a neat note. You can print out the note and then pen additional comments, or you can type the comments directly into the note before your print it. If you are concerned about computer privacy, you may want to print the note and then write in the patient's name by hand.
I welcome you to contribute to the case note library. Once you make a contribution, it will then appear in the checklist in alphabetical order (you may have to click "refresh" on your browser to see your contribution). Please contribute only your original ideas. Do not contribute any copyrighted material.
Disclaimer! Do not assume that using only the case note library will generate an adequate clinical record. It is assumed that you will also make appropriate additional comments. This is a public library of possible case notes. Some therapists may use interventions with which other therapists disagree. While I may edit some entries for grammar or readability, I do not guarantee that any intervention listed in the library is adequate, ethical or appropriate.
